The second step is fine tuning your form. You can change the order, delete fields or add your own fields at this point. The drop down boxes show you the options available.
The tick box on the right of the field indicates that it is mandatory for the user to fill out this box before the form will send. A message will pop up and remind them if they have not filled out one of these fields.
You can add up to four new fields at a time. If you want to add more, just save the form, and four more fields can be added.
Radio buttons, Check boxes and Drop Down Lists are created by entering a list of options separated by a comma in the text box to the right. (see Male, Female for an example.)
Save the form and then send a test form to check the email address is receiving the emails correctly.
